Kent Center, a not-for-profit organization that provides highly personalized 24-hour a day services for adults who have a broad spectrum of developmental disabilities, such as intellectual disabilities, autism, cerebral palsy, and/or physical disabilities. Since 1970, the mission of Kent Center has been to ensure the safety and well being of each individual family member, treat them with respect and dignity and provide enriching opportunities and choices which will enable them to independently live, work and contribute positively to our community to the greatest extent of their abilities.

We are currently hiring for a full-time Program Coordinator to join our team in Chestertown. This individual will assist in the coordination of the interdisciplinary development and revision of Individual Plans (IP's) and monitors the implementation of IP's by agency staff and other professionals.

Overview of Job Role:

  • Works directly with a caseload of individuals to help them achieve individual goals through a meaningful day. 
  • Schedules the interdisciplinary team meetings (TM's), prepares necessary forms and writes minutes.
  • Conducts team meetings at least bimonthly for each individual to guide the team in the development or modification of individual plans (IP's).
  • Ensures that all individual's evaluations are performed according to IP's.
  • Compiles the IP packet to document the TM's, and incorporates any recommended changes.
  • Distributes a copy of the IP's to all persons involved in the individual's program plan, including the individual and/or guardian.
  • Purges individual's files of the previous years data on a yearly basis.
  • Completes administrative forms documenting individual's status and progress and submits to the appropriate administration.
  • Monitors and assists staff in the writing of training objectives and in the writing and revision of Revised 7/16/2018 training methodologies.
  • Reviews staff documentation of individual's progress on a monthly basis.
  • Observes staff in the implementation of IP's.
  • Visits agency sites on a regular basis to assist staff in the implementation of IP's.
  • Writes quarterly reviews of individual's progress.
  • Provides coverage for agency staff throughout the agency when substitutes are unavailable.
  • Arranges and maintains records of respite care of persons eligible to receive temporary residential services from agency.
  • Reviews individual's plans for their accordance with Developmental Disabilities Administration (DDA) regulations.
  • Performs quality assurance reviews of individual's records.
  • Assist with coordination, supervision and transportation for Special Olympics.
  • Maintains awareness of and abides by the applicable laws, regulations, and accreditation standards governing the provision of services to individuals with developmental disabilities.

The ideal candidate will:

  • Support and enforce Kent Center's core values of being organized, caring, respectful, accountable and willing to help.
  • Have two years of case management experience.
  • Have experience working with a DDA population.
  • Enjoy working at an organization with purpose. 
  • Have excellent oral and written communication skills. 
  • Proficient with Microsoft Office.
